For your healthcare needs, Life Image offers medical coverage through through Blue Cross Blue Shield, paying up to 90% of the annual premium. The medical plan is Preferred Blue PPO, which allows for in-network and out-of-network coverage. It is a high-deductible health plan.

The dental plan is administered by Blue Cross Blue Shield and covers the following categories of expenses:

  • Preventive and diagnostic care, like routine exams and cleaning, fluoride treatments for children, sealants for children, and X-rays.
  • Basic treatment, such as simple fillings and extractions, root canals, oral surgery, and gum disease treatment.
  • Major treatment, such as crowns and dentures.
  • Orthodontia for children under age 19.

The vision plan is insured by Blue Cross Blue Shield EyeMed 20/20 and is designed to help make the cost of eye care more affordable. The plan covers annual exams and glasses or contacts.

WageWorks Health Savings Account

For employees who elect to participate in a Life Image-sponsored medical plan, you will be automatically enrolled in a health savings account (HSA). The company will contribute up to $2,000.00 over the calendar year. Individuals can contribute additional monies on a pre-tax basis, up to the allowed IRS limits.

Employees will also be eligible to participate in flexible spending accounts (only if they are not actively participating in an HSA), dependent care accounts, and commuter accounts, in accordance with IRS limits.
